Your Health Information & Privacy Rights

This page describes how medical information about you may be used and disclosed, and how you can get access to that information. Please review it carefully.
Who holds your medical record
GLP is a technology platform. We are not a medical provider, pharmacy, or insurer, and we do not practice medicine. Your care is provided by independent, licensed clinicians affiliated with OpenLoop Healthcare Partners, PC and the other practices participating in its Affiliated Covered Entity (ACE). Those practices are the HIPAA covered entities that hold your medical record and issue the Notice of Privacy Practices governing it. They maintain a single designated Privacy Officer responsible for HIPAA compliance across the ACE, and may share protected health information among themselves as needed to carry out treatment, payment, and health care operations.
Get Life Perfect, Inc. handles protected health information only on their behalf, as a business associate, under a written agreement that requires us to safeguard it and limits what we may do with it.

Information collected on this website that is not part of your care — for example, marketing enquiries or analytics — is covered instead by our Website Privacy Policy.
Your rights over your health information
- Get a copy of your medical record. You can ask to see or receive an electronic or paper copy. A copy or summary is usually provided within 30 days of your request, and a reasonable, cost-based fee may apply.
- Ask for a correction. You can ask for information you believe is incorrect or incomplete to be corrected. A request may be denied, but you will be told why in writing within 60 days.
- Request confidential communications. You can ask to be contacted in a specific way, or at a different address. Reasonable requests are granted.
- Ask for limits on what is used or shared. You can ask that certain information not be used or shared for treatment, payment, or operations. The practice is not required to agree, and may decline where it would affect your care.
- Restrict disclosure when you pay out of pocket. If you pay for a service or item in full yourself, you can require that it not be shared with your health insurer for payment or operations — and that request must be granted unless the law requires disclosure.
- Get a list of disclosures. You can request an accounting of when your information was shared in the six years before your request, with whom, and why. Certain disclosures — including for treatment, payment, and operations — are excluded. One accounting a year is free.
- Get a paper copy of the Notice. Available on request at any time, even if you agreed to receive it electronically.
- Choose someone to act for you. A person with medical power of attorney, or your legal guardian, can exercise your rights and make choices about your information once their authority is confirmed.
- File a complaint. You can complain if you believe your rights have been violated. You will not be retaliated against for doing so.
Your choices
For certain information you can say how it is shared. Tell your care team your preference and it will be followed. You have both the right and the choice to say whether information is shared with family, close friends, or others involved in your care, and in disaster relief situations. If you are unable to state a preference — for example, if you are unconscious — information may be shared where it is believed to be in your best interest, or where needed to lessen a serious and imminent threat to health or safety.
Written permission is always required before your information is used for marketing, before it is sold, and for psychotherapy notes except where the law permits otherwise. If you are contacted in support of fundraising, you may opt out of future fundraising communications at any time, and doing so will not affect your care.
How your information is typically used and shared
Your information may be used and shared, without separate permission, in order to:
- Treat you — sharing with other professionals involved in your care.
- Run the practice — managing care, improving quality, and contacting you when necessary, including appointment reminders by email or text.
- Bill for your services — obtaining payment from health plans or other entities.
- Help with public health and safety issues, and comply with the law.
- Support health research, under the conditions the law requires.
- Respond to organ and tissue donation requests.
- Work with a medical examiner, coroner, or funeral director.
- Address workers’ compensation, law enforcement, health oversight, and other government requests.
- Respond to lawsuits and legal actions, including a court order or subpoena.
Any other use or disclosure is made only with your written authorization, which you may revoke at any time.
Responsibilities to you
- The privacy and security of your protected health information must be maintained by law.
- You will be notified promptly if a breach occurs that may have compromised the privacy or security of your information — without unreasonable delay and no later than 60 days after discovery.
- Where state law provides greater privacy protection than HIPAA, state law is followed.
- The duties and practices described in the governing Notice must be followed, and a copy of it given to you.

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights, by letter to 200 Independence Avenue S.W., Washington, D.C. 20201, by calling 1-877-696-6775, or at hhs.gov/ocr/privacy/hipaa/complaints. You will not be retaliated against for filing a complaint.
Changes
The terms of the governing Notice may change, and any change applies to all information held about you. The current version is always available at the link above, and this page is updated to match.
