Terms of Service

Last updated: August 4, 2026

DO NOT USE THIS SITE IF YOU THINK YOU MAY HAVE A MEDICAL EMERGENCY. In an emergent situation, you can: (i) call 9-1-1; (ii) go to the nearest emergency room; (iii) contact your local crisis center; or (iv) if applicable, call or text the 988 Suicide & Crisis Lifeline.

These Terms of Service (“Terms”) govern your access to and use of Get Lift Perfect LLC’s website and services (the “Service”). By accessing or using the Service, or by clicking to accept these Terms where that option is made available, you accept and agree to be bound by them. If you do not agree, you cannot use the Service.

1. Eligibility

You must be at least 18 years old and located in the United States to use the Service.

2. Privacy

Please review our Privacy Policy for information about how we collect and use your personal information. Uses and disclosures of your protected health information are governed by the Notice of Privacy Practices of the practices that treat you; see our guide to your health information rights for that Notice and a summary of your rights.

3. Changes to these Terms

We may revise and update these Terms from time to time in our sole discretion. Changes are effective upon posting, and your continued use after a change has been posted constitutes acceptance, unless applicable law requires additional notice. We encourage you to review these Terms regularly.

4. The Service does not provide medical advice

THE INFORMATION AND CONTENT AVAILABLE ON OR THROUGH THE SERVICE IS FOR INFORMATIONAL PURPOSES ONLY AND IS NOT INTENDED TO SUBSTITUTE FOR PROFESSIONAL MEDICAL ADVICE, DIAGNOSIS, OR TREATMENT. ALWAYS CONSULT YOUR HEALTHCARE PROVIDER WITH QUESTIONS ABOUT ANY MEDICAL CONDITION BEFORE MAKING HEALTHCARE DECISIONS.

GLPis a technology platform that connects you with independent, licensed clinicians and pharmacies. We are not a medical provider, pharmacy, or insurer, and we do not practice medicine. “Provider” means an independent licensed clinician or professional entity that provides clinical services to you through or in connection with the Service. Clinical decisions, including whether to prescribe, are made solely by your Provider.

Care delivered through the Service is subject to your Provider’s Telehealth Consent, which you must review and accept before treatment begins.

5. Reliance on information posted

We do not warrant the accuracy, completeness, or usefulness of information presented on the Service. Any reliance you place on it is strictly at your own risk. There is no guarantee of results or of the effectiveness of any treatment or care plan.

6. Your account

You are responsible for the accuracy of the information you provide and for keeping your login credentials secure. You agree to provide truthful health information so that Providers can make appropriate decisions.

7. Communication preferences; electronic notices and signatures

By creating an account, you consent to receive electronic communications from us — by email to the address you provide, by text message (SMS) to a mobile number you provide, or by notices posted to the Service. These may include operational notices about your account, such as password changes and other transactional information, and are part of your relationship with us. You may incur data, call-time, or messaging charges from your communication providers. We may also send promotional communications, which you can opt out of at any time using the unsubscribe instructions in those messages.

You agree that notices, agreements, disclosures, and other communications we send electronically satisfy any legal requirement that they be in writing. You agree that using your finger, a keypad, mouse, or other device to select an item, button, or icon while using the Service constitutes your signature, acceptance, and agreement as if actually signed in writing, and that no third-party verification is necessary to establish its validity. You should keep copies of electronic communications from us.

8. Pharmacy partners, fulfillment, shipping, and packaging

The Service may facilitate prescription orders placed by your Provider. Prescriptions are dispensed by independent, licensed partner pharmacies (“Partner Pharmacies”) and/or a pharmacy of your choice, depending on the program and your location. We do not own, operate, or compound medication at any of them. You authorize Providers and the Service to transmit prescription information to one or more pharmacies for dispensing and shipment, as permitted by law. Any prescription order is at the professional discretion of your Provider.

  • Pharmacy transfers. Your prescription may be transferred among pharmacies for fulfillment, availability, pricing, or operational reasons, consistent with law and your instructions.
  • Shipping. Shipping times are estimates and may vary due to pharmacy processing, verification, carrier delays, weather, holidays, or other factors.
  • Temperature-sensitive products. Certain medications require refrigeration or other handling. You are responsible for receiving shipments promptly and following storage instructions.
  • Child-resistant packaging. Some medications may ship in non-child-resistant packaging where permitted. Keep all medications out of reach of children.

Our Partner Pharmacies are:

For questions about your order, your prescription, or your account, please contact GLP at hello@getlifeperfect.com rather than the pharmacy — we can resolve most issues faster and coordinate with the pharmacy on your behalf.

9. State coverage

Services are available in all 50 states and Washington D.C. Medication and product availability varies based on clinical appropriateness, applicable federal and state laws, or other regulatory requirements. Not all therapies are available in all states. Availability is subject to change.

Which treatments and visit types are available to you is determined by the licensing and regulatory requirements of the state you are located in at the time of your visit.

10. Payment, subscriptions, and automatic renewal

By providing a payment method, you expressly authorize us (or our payment processor) to charge it for the total fees associated with your use of the Service, together with any applicable taxes. If your payment method cannot be verified, is invalid, or is otherwise not acceptable, your purchase may be suspended or cancelled, and you must resolve the problem before we proceed. If a payment is not successfully settled and you do not update your payment method or cancel, you remain responsible for uncollected amounts and authorize us to continue billing the payment method as it may be updated.

  • Services are offered on a cash-pay basis and are not billed to insurance. We do not guarantee that amounts paid are eligible for reimbursement by insurance, HSA/FSA, or otherwise.
  • If you enroll in a subscription or recurring program, you authorize us and our payment processors to charge your payment method on a recurring basis until you cancel. Subscription terms, billing cadence, and cancellation deadlines are disclosed at checkout and in your account.
  • To avoid renewal charges you must cancel before the disclosed cutoff prior to your renewal date. See our Cancellation and Refund Policy.
  • Payments may be processed by third parties. We do not store full payment card numbers.
  • Clinical and pharmacy charges may be billed separately where applicable, and are disclosed before you pay.
  • If you initiate a chargeback, we may suspend your account, and collection efforts may apply to amounts legitimately owed.

11. Refunds

All sales are final except as expressly stated in our Cancellation and Refund Policy or as required by law. That policy is incorporated into these Terms.

12. Promotions

From time to time we may offer promotions, discounts, or referral programs. These may be subject to additional terms and eligibility rules that are incorporated by reference. Unless prohibited, those additional terms control in the event of a conflict with these Terms.

13. Acceptable use

These Terms permit you to access and use the Service for your personal, non-commercial use only. You agree not to:

  • Provide false information or impersonate another person or entity.
  • Attempt to access accounts or data that are not yours.
  • Reproduce, distribute, modify, create derivative works of, republish, or reverse-engineer the Service or any of its content.
  • Use the Service in violation of any law.
  • Use any robot, spider, or other automated means to access the Service, or any device or software that interferes with its proper working.
  • Introduce viruses, worms, or other malicious material, or attempt to gain unauthorized access to, damage, or disrupt any part of the Service or its infrastructure.
  • Use the Service in a way that disables, overburdens, or impairs it for others.

If you breach these Terms, your right to use the Service stops immediately and you must, at our option, return or destroy any copies of materials you have made.

14. Ownership and trademarks

The Service and all content included in it are the exclusive property of Get Lift Perfect LLC or its content suppliers and are protected by copyright, patent, trademark, and other proprietary rights. No right, title, or interest is transferred to you, and all rights not expressly granted are reserved.

The GLP name, logo, and all related names, logos, product and service names, designs, and slogans are trademarks of Get Lift Perfect LLC or its affiliates or licensors. You must not use them without our prior written permission. All other names, logos, and marks appearing on the Service are the trademarks of their respective owners, and their appearance does not imply any affiliation with or endorsement by those owners.

15. Changes to the Service

We may update the content on the Service from time to time, but it is not necessarily complete or current. Any material on the Service may be out of date at any given time, and we are under no obligation to update it.

16. Links from the Service

Where the Service contains links to third-party sites or resources, they are provided for your convenience only. We do not control or endorse them and accept no responsibility for them or for any loss or damage arising from your use of them. You access them entirely at your own risk and subject to their own terms.

17. Termination

We may terminate or suspend your access to all or part of the Service at any time, with or without notice, for any reason, including violation of these Terms. The following provisions, and any other provision that by its nature should survive, will survive termination: Disclaimers, Limitation of Liability, Indemnification, and Governing Law.

18. Disclaimer of warranties

THE SERVICE AND ITS CONTENT ARE PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS. YOU AGREE THAT YOUR USE OF THE SERVICE IS AT YOUR SOLE RISK. GET LIFT PERFECT LLC, ITS AFFILIATES, AND ITS LICENSORS, SERVICE PROVIDERS, EMPLOYEES, AGENTS, OFFICERS, AND DIRECTORS DISCLAIM ALL WARRANTIES OF ANY KIND, INCLUDING ANY IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. WHERE YOUR JURISDICTION DOES NOT ALLOW SUCH LIMITATIONS, THIS LIMITATION MAY NOT APPLY TO YOU, AND YOUR SOLE AND EXCLUSIVE REMEDY IS TO DISCONTINUE USING THE SERVICE.

19. Limitation of liability

TO THE FULLEST EXTENT PROVIDED BY LAW, IN NO EVENT WILL GET LIFT PERFECT LLC, ITS AFFILIATES, OR ITS LICENSORS, SERVICE PROVIDERS, EMPLOYEES, AGENTS, OFFICERS, OR DIRECTORS BE LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, EXEMPLARY, OR PUNITIVE DAMAGES, UNDER ANY LEGAL THEORY, ARISING OUT OF OR IN CONNECTION WITH YOUR USE OF, OR INABILITY TO USE, THE SERVICE, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. YOUR SOLE REMEDY FOR DISSATISFACTION WITH THE SERVICE IS TO DISCONTINUE USING IT.

Certain jurisdictions do not permit the exclusion of certain warranties or the limitation of liability for incidental or consequential damages, which means some of the above limitations may not apply to you. In those jurisdictions, the foregoing exclusions and limitations will be enforced to the greatest extent permitted by applicable law.

20. Indemnification

You agree to defend, indemnify, and hold harmless Get Lift Perfect LLC, its affiliates, licensors, and service providers, and their respective officers, directors, employees, contractors, agents, suppliers, successors, and assigns from and against any claims, liabilities, damages, judgments, awards, losses, costs, expenses, or fees (including reasonable attorneys’ fees) arising out of or relating to your violation of these Terms or your use of the Service.

21. Governing law

These Terms, and any dispute arising between you and Get Lift Perfect LLC related to your use of the Service, are governed by the laws of the applicable state and the United States, without regard to conflict-of-law rules.

22. Waiver and severability

No waiver by us of any term or condition set out in these Terms is a further or continuing waiver of that term or any other term, and any failure by us to assert a right or provision does not constitute a waiver of it. If any provision is held invalid, illegal, or unenforceable, it will be eliminated or limited to the minimum extent necessary so that the remaining provisions continue in full force and effect.

23. Geographic restrictions

Get Lift Perfect LLC is based in the United States. Access to the Service may not be legal for certain persons or in certain countries. If you access the Service from outside the United States, you do so on your own initiative and are responsible for compliance with local laws.

24. Entire agreement

These Terms, together with the policies incorporated by reference, constitute the sole and entire agreement between you and Get Lift Perfect LLC regarding the Service and supersede all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral.

25. Contact us

Feedback, comments, requests for support, and other communications relating to the Service should be directed to hello@getlifeperfect.com.
