You have the right to
- Ask questions about the potential health risks of this program, its nutritional content, and its psychological-support and educational components.
- Receive an itemized statement of the actual or estimated price of the weight-loss program, including extra products, services, supplements, examinations, and laboratory tests.
- Know the actual or estimated duration of the program.
- Know the name, address, and qualifications of the dietitian or nutritionist who has reviewed and approved the weight-loss program, in accordance with section 468.505(1)(j), Florida Statutes.
Weight loss and dieting information
Ask the person providing or selling you weight-loss advice or diet products, medications, or formulas about their qualifications and training in nutrition and health.
Long-term weight control is the safest and most important goal of any diet program. Permanent lifestyle changes — such as eating nutritious foods, calorie control, and increasing physical activity — help promote long-term weight loss according to medical experts.
How to exercise these rights
Email care@getlifeperfect.com and we will provide an itemized price estimate for your specific program, its expected duration, the qualifications of the clinicians and the dietitian or nutritionist who reviewed it, and answers to any questions about health risks — before you are charged anything.
Membership, medication, laboratory tests, and any other extras are itemized and disclosed to you in full before you are charged — nothing is billed that you have not seen first. Treatment is prescribed only when a licensed clinician determines it is appropriate, and there is no guarantee of results.
Residents of New York
These rights are provided under N.Y. General Business Law § 642, which gives you the right to ask questions about the potential health risks of this program or product, its nutritional content, and its psychological-support and educational components; to know the price of treatment, including the price of any extra products, services, supplements, and laboratory tests; and to know the duration of the program recommended to you.
New York residents also have the right to request an itemized price list for laboratory services.
